In 2015, I gathered contact information from the entities I encountered after losing my husband, Lt Col R. Tracy Mead, Jr (ret), to pancreatic cancer. The result is the Spouse Survivor Guide, a copyrighted booklet which has been distributed locally to funeral homes, clergy and hospice. Since I live in St. Louis, MO, some of the contact information may need to be converted to your state agencies. However, much of the information is universal such as Social Security, Financial, Military, etc.
Contents include:
- General, Life Insurance
- Social Security, Health/Dental, Employment
- Will/Trust, Vehicle Title, Dept. of Revenue
- Bank Accounts, Credit Card Companies
- Credit Reports, Taxes
- Military/Veteran Contacts
- Notes/Things To Do
- Personal message from Debbie
Booklet Info – Take it or Leave it
**This is a guide, NOT a definitive how-to. Look at it as a non-fiction book. Take what you need and leave the rest.
